The subject is Earl Sande, a jockey whose name appears in the National Photo Company's negative data, though that identification is unverified. Sande, born in 1898, was rising through American racing around the time this glass negative was made, before he became one of the most celebrated riders of the 1920s and won the Kentucky Derby three times. Glass plates like this were a standard way to record sporting figures in the years after World War I. The streaks and blemishes on the surface come from the coating deteriorating over time, not from the original exposure.
