Fort Sam Houston, on the north side of San Antonio, Texas, was a major Army post in 1919, and its base hospital was one of the largest medical facilities the Army operated in the country that year. The First World War had ended in November 1918, and the hospital still cared for soldiers recovering from wounds, gas exposure, and influenza, the pandemic that swept through military camps in the fall and winter of 1918 and 1919. San Antonio had grown into a city of well over 160,000 people by 1920, with its economy built on the military, wholesale trade, and a large Mexican American community that gave the city much of its character. Fort Sam Houston had been a permanent post since the 1870s, so the hospital's buildings stood among a long line of Army structures that residents of the city knew well.
