In 1906, the Middletown State Homeopathic Hospital stood in Middletown, the Orange County city in the Wallkill Valley of New York's Hudson Valley region. The state had opened the institution in the 1870s as a homeopathic asylum for the insane, treating patients with the lower-dose medicines of that school alongside its custodial care. Middletown itself was a railroad and market town on the Erie Railroad's main line, and its economy depended on rail traffic, farming in the surrounding county, and local trade. By the early 1900s, state hospitals across New York were struggling with crowded wards, and Middletown's institution was one of several that housed a growing number of patients. Residents would have known the grounds as a large, self-contained campus on the edge of town, with its own farm and workshops.
