The title pairs two calamities that struck Johnstown in 1889. On May 31, the South Fork dam above town gave way after days of heavy rain, sending a wall of water down the Little Conemaugh valley that killed roughly 2,200 people. The next day, debris jammed against the Stone Bridge caught fire, burning wreckage and victims alike. The title's pairing of flood and fire reflects how the disaster was reported and remembered.
