The title casts Philadelphia's medical schools as a factory, a figure of speech aimed at the large number of physicians the city turned out in the 1890s. The lithograph appeared in *Puck*, the New York satirical weekly known for its color cartoons. Philadelphia was then a major center of medical training, and critics often questioned whether speed and volume came at the expense of quality. The record gives only a rough date, so the cartoon's specific occasion is uncertain, but its satire fits that debate.
