I've only eaten there once, but in SLC, there was a restaurant (about 3 years ago) called "The Five Alls".  I remember it being pretty good.

I'm definitely no help on the others.

Try Lamb's. It's downtown. The food's not fancy, just really well-prepared nice-downtown-diner type food. I had the chicken fried steak. It was really much closer to a very good schnitzel. Who would complain?
