The best hamburgers I've found in San Francisco were at the Washington Square Bar and Grill in North Beach.

Without knowledge of Mel's, I'll admit that I'm partial to the burgers at:
1) The Los Altos Dairy Belle (somehow different from other Dairy B(H)elles
  I've tried);
2) Clark's
3) Kirk's (Palo Alto/California Ave.)

My fav is (was) Johnny Rockets in San Jose. Then I found Taxis in San jose (Oakridge shopping center). I have a new favorite place...

I agree with Steve Fenwick:
Clarke's Grill (one in MV, one---I think---in Pleasanton, and some others around the bay). Expensive, but worth it.
